Young adulthood is a critical period for intervention in the prevention and treatment of substance use disorders. Individuals between the ages of 18 and 24 often experience significant transitions in education, employment, housing, and healthcare, which can increase vulnerability to substance use and create barriers to accessing support.
The 18-24 Young Adult Substance Use Disorder (SUD) Program at Foundations to Freedom allows us to meet young people where they are, provide support, and help them build lives rooted in recovery, resilience, and purpose.

peer support specialist

Each client receives a Case Manager who will develop a wellness and recovery action plan based on the client’s specific recovery needs.

support groups

Daily mutual support group meetings provide encouragement, accountability, and connection as clients navigate addiction recovery.

goal Setting

Clients work closely with their Case Managers to set and attain new goals for each stage of their recovery journey.

recovery sponsor

Each client is encouraged to get and retain a sponsor to provide guidance, support, structure, and accountability during the early years of recovery.

TRANSITION Plan

Case Managers work closely with each client to help them create a plan for life after living in a recovery residence.

ongoing support

Foundations to Freedom offers all alumni 6 months of peer support and Case Management at no cost, plus access to low-cost housing.
